Support through phases 2, 3 and 4
Where RISE fits in your recovery.
RISE is designed for life after hospital: helping you prepare for cardiac rehabilitation in Phase 2, supporting your learning during Phase 3, and keeping you connected in Phase 4.
Cardiac rehabilitation is more than exercise.
Cardiac rehabilitation is a personalised programme of exercise, education and support to help you recover after a cardiac event or procedure, or live well with a heart condition. It brings together physical recovery, emotional wellbeing and healthier everyday habits.
Education is a core part of recovery. Understanding your condition and risk factors helps you make informed choices, ask your healthcare team useful questions and put what you learn into practice.
Phase 1 · In hospital
Your hospital team leads your treatment, early recovery and discharge preparation. RISE has a smaller role here: you may discover it while planning for home, but its main support begins after discharge.
RISE education and peer support alongside your care · Phases 2–4
-
Phase 2
Early recovery at home
After discharge, as you prepare for your structured rehabilitation programme.
How RISE can help
- Understand your heart and recovery through clear, accessible education.
- Learn what to expect from cardiac rehab and prepare questions for your team.
- Find reassurance and connection with people on a similar journey.
-
Phase 3
During cardiac rehab
Your structured programme of exercise, education and support with the rehabilitation team.
How RISE can help
- Reinforce your learning with presentations on heart health, wellbeing and daily life.
- Ask general recovery questions in live sessions with Nikki, alongside your team’s individual advice.
- Share experiences and encouragement with peers between your rehab sessions.
-
Phase 4
Long-term recovery
Maintaining healthy habits and activity after your structured programme, including community exercise where appropriate.
How RISE can help
- Stay connected through RISE Together’s forum, peer chats and optional buddy support.
- Keep learning through shared resources and monthly live sessions with Nikki.
- Find encouragement to maintain the healthy habits developed during rehab.
RISE complements your cardiac rehabilitation and ongoing care. It provides education and peer support, not individual medical advice, exercise prescriptions or supervised exercise.
Understanding the phases of cardiac rehabilitation
These four phases describe a common UK recovery pathway. Timings and services vary, and phases can overlap. Your healthcare team will explain your own pathway and the support available locally.
